WHAT: Guidance on how to complete the referral form, initial assessment and core assessment from the resource Framework for the Assessment of Children and their Families. 

WHO:  Social and community workers referring a child for assessment or a service, and those undertaking the assessment of a child and/or family.  It can also be used for training purposes. 

WHERE:  The documents accompanying the Framework for the Assessment of Children and their Families are all based on the UK context and will require some local adaptation. This guidance is only relevant if the Framework Referral and Initial Information Record, Initial Assessment Record, and Core Assessment Record are used.  

WHY:  Assists the practitioner in understanding what questions to ask when assessing children and their caregivers, and how to record this information in a clear and consistent manner.
