WHAT: Age specific forms for collecting a large amount of information from a variety of sources in order to obtain a thorough understanding of the child’s needs, the parent’s capacities, and the impact of family and environmental factors.

The forms utilize the assessment structure outlined in the document Framework for the Assessment of Children and their Families and should be used with the accompanying guidance:  Framework for the Assessment of Children and their Families: Guidance Notes and Glossary for Referral and Initial Information Record, Initial Assessment Record, Core Assessment Record

WHO:  Social and community workers completing an assessment of a child and their family.

WHERE:  Relevant globally as a template.  It would require some changes in order to fit specific contexts.  

WHY:  This is a very helpful and comprehensive template for recording the complex needs of a child and determining the appropriate response.  For non-complex cases or one-off referrals, the form Initial Assessment Record should be used.
