Referral Mechanisms

 

A referral mechanism is a cooperative framework designed to ensure clients (children and/or caregivers) access holistic support and services most suitable and appropriate to their needs and in accordance with their rights. Referral mechanisms facilitate referrals: the formal process of requesting a service for a child,young person or family from a government agency or third party provider. The request is usually made in writing using an agreed referral form or procedure.
