The Teenage Mother project is an intervention model to support teen mothers, developed by Hope and Homes for Children (HHC) in Rwanda. The documents provides an in-depth analysis of the challenges faced by teen mothers, the prioritization of causal factors, and the implementation of the Active Family Support (AFS) model to address these challenges.

These guidelines provide minimum standards to be adhered to in the provision of Child Welfare Programmes; The guidelines will also provide a framework within which state and non-state actors shall develop, design, and implement childcare and welfare programmes to enhance child rights, strengthen family and community-based care.

These national guidelines provide a roadmap of activities to guide the state and non-state actors in Kenya to streamline the transitioning of care systems, children and institutions in the country.

Based in Uganda, Child’s i Foundation successfully transitioned their residential programs before deciding to expand their vision and seek to bring about care reform to the entire district of Tororo. Partnering with the district level government and several CCIs, Child’s i Foundation carried out an extensive analysis and assessment of the Tororo region which provided information to guide the ensuing transitions of CCIs throughout the region.

Engaging with key stakeholders is an essential part of any transition and must be handled with tact and wisdom. Located in South Africa, the organization Beautiful Gate began its ministry to protect street children and later grew to provide residential programs for children in need. Yet, as they began to learn more about the needs of children in families, they decided to shift away from residential care and expand their services to include the families of the children they served. This case study summary explains how Beautiful Gate communicated these changes with donors.

Overtime, and after realising the full cost of running even a small residential program, and witnessing first-hand the developmental gains made by children once placed in foster care, Child’s i Foundation made a decision to fully transition and phase out their residential care program entirely.

This Handbook is a summarized, simplified version of the Case Worker’s Guidebook for Case Management for Reintegration of Children/Young Adults into Family- or Community-Based Care. It provides an overview of the principles and practices of case management for reunification and placement of children/young adults outside of parental care (e.g., street-connected children/young adults or from Charitable Children’s Institutions and Statutory Children’s Institutions) into family- and community-based care, up until sustainable reintegration is achieved. The Handbook aims to provide an easy and quick reference to critical information and “how to” about case management for reintegration.

This practical guidance is for anyone working with children at risk of entering, already living in, preparing or having already left care. It discusses why and how to support children who are at risk of or who have already experienced adverse experiences that might lead to distress or trauma.
